WAIVER AND RELEASE OF LIABILITY

I know participating in athletic activities and other HFFLH events is a potentially dangerous or hazardous activity. I assume all risks associated with my participation in such events, including, but not limited to injury, falls, contact with other participants, conditions of weather including rain, lightning, ice, snow, hot temperatures and cold temperatures, the conditions of the field, etc., all such risks being known and understood by me.

Having read this Waiver and Release of Liability, knowing the risks associated with my participation in athletic activities, and in consideration of being permitted to participate in the HFFLH, I for myself and anyone entitled to act on my behalf, waive and release the HFFLH, its officers, volunteers, board members, representatives, and members from all claims or liabilities of any kind arising out of my participation in the HFFLH though that liability may arise out of negligence or carelessness on the part of the persons identified in this Waiver and Release of Liability. I further release the HFFLH, its officers, board members, representatives and members from any claim whatsoever because first aid, treatment or service rendered to me in connection with my participation in the HFFLH.

By agreeing this waiver, I grant full permission for the use of my likeness in photographs or other media for any publicity and/or promotional purpose without obligation or liability to me.
